Bone remodeling is a lifelong process that involves the removal of old or damaged bone, followed by the deposition of new bone material. This process is essential for repairing old damaged bone due to daily physical load and preventing aging effects and its consequences. Impaired bone remodeling can result in excessive microdamage burden, accumulation of advanced glycation end products (AGEs), and hypermineralization.

In osteology, bone remodeling or bone metabolism is a lifelong process where mature bone tissue is removed from the skeleton (bone resorption) and new bone tissue is formed (bone remodeling). The skeleton then completely regenerates itself about every 10 years, with most of the adult skeleton replaced about every 10 years. Both genes and the environment contribute to bone health.

In adults, this process takes seven to ten years. Bones stop growing in length between the ages of 16 and 18, but the total amount of bone tissue remains. Up until about age 40, all the bone removed is replaced. Most people reach their peak bone mass around age 30, and after that, bone remodeling continues, but slightly more bone mass is lost than gained.

Before age 40, adults can expect to experience bone loss due to an imbalance in “bone remodeling”. Medication stimulates bone remodeling rather than slowing down bone breakdown, but use is limited to two years at this point. If bone remodeling came to a halt, the accumulation of microfractures would cause mechanical failure of the skeleton in about 2 years. In adulthood, each remodeling unit is balanced, lasting between 90 and 130 days.

  • So strange. So I’m a 65 yr old women. Quite active and healthy. I broke my 5th metatarsal. Spiral fracture. Bones well aligned. 3 drs told me it would take a minimum of 6wks to heal. Possibly 8 was up to 12! 😩. No cast just a boot and non bearing wt. I religiously elevated and iced the area 3 times or more a day. Cut out alcohol, took a calcium vitamin d supplement. Ate lots of dairy, protein, fruits and vegetables. Did not worry about weight gain just loaded up on things like berries, bananas, nuts, plain Greek yogurt, eggs, meat and protein drinks. Also wore support knee high. Shockingly after 3 1/2 weeks my bone healed!!! Orth surgeon said I could walk on it as long as it didn’t hurt. So I gave up the boot and scooter and wore a supportive shoe. Walking with no pain at all!!! I couldn’t believe it. At the end of the day I still elevate it just out of habit and it feels good to relax that way. Tuesday will be 6 wks and I will get the X-ray to be sure it’s healed. Wish I could have X-rayed it at 3 1/2 weeks just out of curiosity. Anyhow I’ve been walking my dog twice a day on a flat surface and back to aqua aerobics 3 X per week. Have you ever heard of such quick healing in a women my age? I’m totally grateful but really don’t understand how this could be. Dr said 6wks usually minimum to heal that bone weather ur 65 or 25.

  • Since I searched and searched trying to find information on my injury and wasn’t much out there, I came back to articles I watched to share with everyone how my healing journey has been like. I fractured my plateau tibia on the 24/09/23 playing netball. I was rushed to the hospital because of the swelling on the knee (the pain was too much). X-Ray was done but everything seemed okay. I was then sent for an MRI scan and it showed a non-displaced fracture to the plateau libia. The Dr ordered me to wear a hinged brace and crutches (I was using crutches already from the day I got injured). I had X-rays done again on the 1/11/23 and the results were positive. No surgery needed. The same day of the 2nd X-Ray, The Dr told me to stop using both the crutches and a hinge. I have been limping around for the past 5days😊. The swelling is still there and a bit uncomfortable but the Dr said it is normal and will disappear with time. My next appointment with the Dr is in 5weeks. Additional info: I stopped taking pain meds 2weeks after the injury because I was not feeling that much pain but continued taking an inflammation pill until I ran out of what the Dr had prescribed. I know how these injuries can be frustrating especially the fact that, information out there is not enough. All the best

  • Fractures I have had. 1. Compression fracture T12 / 4 month’s P. T. Kyphoplasty 2. Trimalleolar comminuted ankle fracture :with 2 screws in tibia and plate with 6 screws fibula. Healing time 6.5 months 6 months P. T. 3. Forearm radius and Ulna displaced fracture with long plate in ulna with 8 screws and longer plate with 12 screws. This was the second surgery as within 2 weeks bone separation again from plates. Took 4 + months to heal 4. Broken 3rd toes proximal phalanx. Non displaced fracture. Only a month. Hard post op sandal.

  • I got in a car wreck in September of last year. I shattered my ankle and broke my fibula. But 4 months later my bone is still not healed. My doctor ordered a bone stimulater to help my bone to heal faster. He said if that doesn’t work. Iwill have to have another surgery to take the plate and screws taken out. Then he would have to put a nail up through the bottom of my foot.

  • When I was 14, tib/fib break that had to be set. Surgery that day. No weight bearing for 12 weeks. Hard cast. 12-16 weeks, I was able to go back to full weight bearing, but still had the hard cast. My calf muscles shrunk so much. Now at 36, just had patella femural partial knee replacement and an osteotomy to fix alignment. The osteotomy involved getting a wedge cut out of my femur, closing that up, and setting it. No weight for 4 weeks. Just started 2 weeks partial weight, then go full weight bearing. After 12 weeks, I get the other knee done.

  • I got t into a car accident on June 30 my and had surgery on my left femur and right arm I got all help from home health nurse and PT that provided from the hospital I’m so thankful For everything. They said it I have one more weeks to be able to put a little bit of pressure on leg so I can start walking a little bit. Thanks for sharing your articles.

  • Watching this lying on the hospital bed with a crack on my pelvic bone. I had an aluminium plate and screw fitted in, the doc said, it’s not permanent. It’s just so that my bone stays aligned and will remove it after an year. I have got 17 stitches and this is the second day after my surgery. Hope to recover soon and go back to my normal life.

I fractured my tibia and fibula 9 weeks ago now and the bones weren’t displaced they say i have a little spiral fracture on both the tibia and fibula about 6 inches about my ankle and believe it or not I didnt need pins or plates just simply a cast now what I’m wondering is I’m in my cast still 9 weeks in but I have a little shoe that attaches to my cast and I’m putting my full weight through my right leg now with absolute zero pain would you reccomend me stay in my cast for the 12 weeks they want me too (I’m into week 9) or should I get the cast off and go into my boot which they are going to give me at week 12 anyway? It’s so frustrating being stuck in this thing when in walking on it without crutches now I just want to get back to work, this really challenges your mental health having to depend on people helping you out really drives me nuts.   • I’m 55 yrs old, & this is my FIRST time breaking a bone. 2 weeks ago I lost my balance in my back yard, fell backwards, & tried to catch myself. Didn’t work out very well lol I’ve got a stable fracture if my radial styloid something or other lol. It hurt like HELL, when I fell & tried to get myself off the ground. It’s hurts any time I try to open something like a ziplock..omggg.. its such a pain in the @ss. I’m finally able to at least put a ponytail in my hair, and also to button my jeans. My dr says my healing time may be longer because my xrays show I have osteoperosis 😮

  • 18 days with non dislocated fibula ankle fracture wondering can I ride a stationary bike? If so how long suggested? I’m in a boot (I had a splint from ER, but now in a United Ortho AIR Cam Boot, self paced, no doctor). 44 years old, healthy. Down to slight swelling/to none; maybe taking 2 Tylenol 650mg extended release pills some nights just to rest from my own mind and maybe a level 3 pain/uncomfortable feeling based on the way I elevate with the boot off at night, but still wrap in the air liner with pressure inflated.

  • I just broke my tibial plateau one week ago today. It’s a type 2 with 1.8mm displacement. I was hit by a Doberman running full speed. I’m 58yo and I knew immediately that it was broken. surgeon has put me in a hinged brace,no weight bearing with ROM and some strength exercises. I’m hoping it heals quickly as I’m going stir crazy!

  • It helps a lot, hey my middle finger broke because I fell down the stairs and I got up and the doctor just put finger braces on it. The orthopedic doctor has not seen it yet because it is still scheduled for the next day.   • This was very informative thank you! As I’m typing this I have a broken left arm. I got bucked off a horse and it felt like it was numb but also tingly. I’m going to a specialist this week but I got x-rays at the ER and,thankfully, it’s a stable fracture. I broke my left funny bone (forgive me if I’m wrong I’m pretty trippy because of the meds lol) which is apparently rare in children. The specialist in my town won’t see me because I’m to young so we have to drive to a different one. For anyone wondering im ok other than my arm and new trauma. I am however pissed at the idiots that spooked the horse I was on. I broke my left wrist by falling off a bike and it broke so perfectly that I dint need a cast. It dint hurt but felt sore when I put pressure on it. It healed within 3 weeks (I wore the cast s day longer though) and I still did ridding lessons. Also I had a horse bolt on me but I somehow managed to stay on but my instructor fell of hers. Funny story about the first one. I was wearing a shirt that said “a horse will never break your heart only your bones, all good” at the time so pretty ironic. Also completely different reaction from the staff when you tell them you fell off a bike and telling them you fell off a horse. I’d also like to apologize to the door I ragged at absolutely for no reason today and for giving my trainer a heart attack because I dint move for a good minute when I fell off the horse and she probably thought I was dead or unconscious. But I’m 13 right now and I’m honestly surprised I haven’t died or gotten a concussion yet

  • I have a boxer’s fracture and have been in cast for 3 weeks now and when my dr rmove my cast to check my progress it looked like my 5th metacarpal isnt allined properly and i doubt my dr didnt do any reduction and my anxiety is killing me rn, my fracture still hurts and didnt look like it has progressed little bit.

  • I broke my Fibula about 6 months ago, I don’t remember the terms but it was bent and about half way broken pretty much and the told me I won’t probably need surgery to straighten it back out. After 6 months I still experience pain after running at a level about a max of 3 pain wise. Should I get another X-Ray to see if it healed properly or wait longer? Also for extra info I’m 15 but I’m not sure if that’s to late for more speedy bone recovery.

  • Very informative article. Thank you. Hit the sidewalk curb with my bike downhill and swan dived into the pavement. I got a deep wound in my chin that needed stiching but other than that I got away from it with only surface wounds and a broken thumb. I got driven into the A&E and the doctors gave me 5-6 weeks untill I can potentially remove the cast. Provided that the bone looks healed on the x-ray. 4 weeks left and I just can’t wait!

I fractured my fibula on 3-10, I ended up going to emergency two days later and they put a splint on it that was worthless after 6 days and i cut it off and used a old aircast for a few days till my appointment. I debated between a cast or a aircast. I took the aircast which was the third option. Kaiser doctor’s first recommendation was surgery, which didn’t make sense because it was a clean fracture and it was healing nicely. The cast seemed like a death sentence, so I took the last option. I was wondering if I made the right choice. The aircast has the benifit of being able to ice the area. I’m going into my third week and most of the swelling and brusinis gone. I’m still wondering if I made the right choice.

  • i broke my left ankle, im in a cast for almost 4 months. My doctor told me that i nedd to practice 25% of my weight. They cant decide if i need to go under operation or we will wait if my bone was healing and will go back again to a normal position without the needs of operation. Is that possible to get my bone back to normal without operation a walk again a normal walk ?? only just mild exercise, eat a lot of food contains a high calcium ??

  • So I got bucked off a horse August 3rd, after going up in the air significantly- full somersault- I landed on my right foot (a little to the outside of the foot) very hard. I felt immediate, searing/burning pain, pretty intense. I writhed around like a wounded snake for a a minute or two, then got up to go catch my horse. It hurt, but I could put weigh on the inside of the foot and sort of hobble along like Quasimodo. Managed to catch the horse and put him away, then went and took copious amounts of Advil and iced my ankle. (Pain was worst on the outside bony part of the ankle.) My ankle swelled up significantly and had a lot of bruising, even down into the heel and toes, quite hideous looking. Because I am a “crazy horse woman” I dragged my leg around for over week before my horse vet volunteered to xray it. Of course, she told me “GO TO THE DOCTOR!” I had an oblique displaced fracture of the fibula. Went to ortho absolutely certain I would need surgery… but he told me he wanted to put a cast on it and have me stay completely off of it for a week and re-X-ray. (Cast hurt like hell, way worse than dragging it around broken had.) After a week, the bone had miraculously re-aligned! No surgery! I was in the cast 3 weeks, went back for another X-ray and ortho said he thinks the break is now in the beginning stages of calcification. He wants me in a walking boot for another 3 weeks. I am (mostly) staying in the walking boot outside, but I take it off in the house. My ankle feels pretty good overall, minimal pain, a little bit “jello-like” where the break is.

  • Hey, I fractured my calcaneus bone 9 weeks ago and the doctor said after 8 weeks I would be able to walk without pain, but I’m still feeling pain. It was a small avulsion fracture on the calcaneus bone. I’m 15 years old. I was just wondering if it is possible that I’m already healed but I still feel pain and any advice going forward?

  • Had an accident crhsitmas week fractured my left heel bone and still can’t move my foot. However it doesn’t hurt as much as it used to The first week.. my bone did not misaligned i fractured my c6 and C7 disk as well also no misalignment.. wearing a neck brace and a boot they said 6 to 8 weeks I work on my feet and I also fish a lot… would 8 weeks be enough

I’m 25 about to turn 26 I’m 12 weeks post surgery things get better after week 8-9 I’m finally walking with 1-2 crutches and I’m just grateful to be alive.   • Hello sir, i had an small accident and 5th metacarpal was fractured, and it’s just in position, doctor said, no need for k-wire, just split is enough for 6weeks. 2weeks back split is removed, now 2months completed, but fracture lines still visible, how long it will take to go off crack line on x-ray, and I’m afraid of do physiotherapy, because when I’m doing physiotherapy some pain on fracture site.   • In 1972 I will stop by vehicle. I’m broke femur the tibia my pelvis, my wrist on my jaw. And we never put a single piece of metal in my body. I have on me is a total of eight months and it actually cut the Kisco off to secretaries of time to reposition my leg. I was given all types of lifetime inspections after I have the cast removed. Well years after the cast was removed, I started playing ice hockey eventually play 250 professional games in Toronto and this day. My leg does feel different but I was talking restrictions were a little ridiculous.

  • I broke my left collarbone 2 weeks ago while skiing. It’s noticeably misaligned, but is still, although barely, connected on the bottom side. Never felt pain during the two weeks, not even when breaking it. Only when putting pressure in it it hurts. I can also still do almost everything with my left arm, lift it up high, carry heavy things and play the piano. It’s weird, knowing it’s broken but not having any pain.

  • I was in a car accident on aug 12th I don’t remember much but waking up screaming in the hospital. The doctors told me I had a displaced comminuted fracture femur.   • My mother is 57. She just fell a week ago and suffered a comminuted spiral fracture of her humerus. Because she’s a heart disease patient who had a heart attack 2 years ago she’s scared of surgery. Ortho said it looks stable now but we’re due in 2 weeks with new X-rays to see if the bones are still in place. They left her in a sling for the time being. Is this normal? She doesn’t have insurance but I’m capable of paying out of pocket for procedures and care for her though I’m worried because of her lack of insurance they may not want to care for her as well as they would someone else. Any advice?

  • Eek no wonder I have a lot of pain I am putting weight on my broken ankle bone. It’s hard to do stuff when you don’t get no help sadly. Seriously hate wearing my boot and using crutches isn’t for me so I have a knee walker.   • Im an 30 year old male I was hit by a car going 50 that ran a red light while I was crossing the road luckily only my left leg was broken plus some road rash i now have a metal plate and 9 screws in holding my tibia together after the surgery but the doctor told me 2 months before I could start walking again an put any pressure on the leg I’m currently 3 weeks in and I’m walking just fine no pain no problems F**cker still won’t let me go back to work smh 😤

  • I had a 5th metacarpal fracture on the last week of January. Had open reduction fixation surgery with 4 pins first week of February, due to the bone breaking in 3 pieces. Went back to work without restrictions mid april so almost 2 months recovery but my grip strength was at 50% at the time and was working toward strengthening it. Then…..first week of june i break the same bone…but this time a displaced fracture. My Dr did a closed reduction, and 3 pins. Lets see how long it takes since is a refractured of the same spot withing 4 months.

I had a fracture in my fibula that there wasn’t any need of surgery to re locate it and I got a boot from the doctor to keep it and now I finished my 8th week and the bone is healed but if I try and jog/run it or try to squat on my toes I feel pain in the calf and my question is how much longer it that going to be for? And if it’s normal, I’m also exercising it almost every day. Thank you

  • Thanks for the information… I met an accident and my right hand shoulder bone broken slantly… Doctor said it will take 2 months to recover… They just tied up with a pop bandage… I’m feeling like the bones are moving inside my shoulder this is 11th day from accident. It’s been 7 weeks and my doctor is not going to like it when I tell him I haven’t been wearing my boot and didn’t finish therapy since I cant work I don’t have the co pay. I need to get serious about healing myself!

  • Broke my humerus in three areas including the head from a motocross crash. Had ORIF surgery. Bone healing is the easy part, it’s regaining strength and range that is slow and difficult! This is definitely an injury u want to do as your doctor and physiotherapist advise and do your at home exercises! I’m 7 weeks in and my range is still only halfway. Curious what is it about rice for bone healing? I just took vitamin D and calcium supplements.

  • Hii….i have avulsion fracture in 5th metatarsal…and my doctor didnt gave any plaster ….he at first said i need to get plaster in the next week but later there was another doctor who addressed me and he said to only keep the boot slab for 5 weeks more ……can u plz tell me how long eill it take to heal im 22 yrs old

  • Id appreciate your advice as my doctor diddnt give me much advice I have a c6-c7 transverse process & spinious process fracture minimally displaced on c7 undisplaced on c6 I am at week 3 of recovery. Can I do any exercises yet to strengthen the muscle? What exercises could I do after the 8 week healing time provided I am signed off to do so?

  • I had a fractured toe two unrelated injury hell couple of I guess the year to what my room like 2 years apart but I mean that wasn’t like it was one of my smaller toes and they just told me to wear the the steel toe shoes all the time and it would feel and what I mean this is really interesting how the body regenerates itself for the proper nourishment and treatment

  • I broke my right forearm radius and ulna, it was an open fracture. Unla has some gap not perfectly aligned after surgery it had a small piece held with screw. It took 7 months for that gap in ulna to have callus. radius joined in within 6 weeks. I had painful nights initial 5 months was horrible. Is this really bad case ? I don’t know if I suffer from anemia but would that be causing delayed union how to speed up recovery and will I ever be able to do strength training in that arm like other forearm.

  • Hey doc i really need some help. I broke the base of my 5th metatarsal of my left foot. Its an Avulsion fracture and the tuberosity or atleast a small part of it looks out of place from the xrays, Doctors put me in a cast for about 2months, But it didnt reunite. It still is the same way as before, im afraid it might heal in that incorrect position, Should surgery be considered? Well, it looks to me like its displaced but cant say for sure as doctors here dont talk that much neither explain whats going on to us patients, they’re too arrogant Im afraid im gonna have lifelong problems cuz of this, please can i get help somehow?

  • I punched the door or wooden door broke my hand with a stress fracture know the surgeon is saying oh the bones are lining up properly blah blah we need to do a surgery the doctor said it looks normal I’m still going to go get a second opinion but asking you personally do you think of this place bone would for sure need surgery or only if it’s extremely bad

  • Not sure if you still reply to comments here but wanna ask anyway. I fell at work and broke my mid humerus, displaced but not extreme, just 1 place and was put together nicely with rod and screws. 2 weeks after surgery now and im off off pain killers and already gained about half my rom back. I can use my phone already and computer, do basic things like cooking etc. I started physical therapy slowly recovering. Now the thing is.. My life has been fitness and sport for the past 10 years, my work involves having to go to the gym, my whole life past years gym has been extremely important. I have read scary things that it can take a year untill i can lift weights again or even do physical work/job? I dont get it, some articles say 3 months, some say half a year, some even a year or even 2 years. Will it really take a year until i can workout normally again? Like even just with moderate weights? Im so depressed about this, im trying to do whatever i can to speed up recovery. Eating healthy and taking vitamin c d3 k2 e, i eat plenty of high protein and calcium, magnesium and zinc too. Im willing to add growth hormone and bpc157 if i have to. Anything to speed up the process..

  • I broke my thumb I forgot what it’s called but it’s the one longest thickest one, They told me didn’t have to have surgery.. but I’ve had 3 cast on because they were not put on so great. My cast that I have on now I feel may be a little loose. I was wondering if it is possible to dislocate it or do something bad to it while it’s in the cast?

  • I just had a street fight and broke both hands on someone who assaulted me. Great article! I guess I turned out pretty well in the end. 😂 Compared to him. Keep your hands to yourself people…. Because i defend myself 😂 left hand still is usable with a few teeth marks but the right hand looks like a boxing glove 🥊 😂. It’s in a brace. I have a comminuted fracture of the neck of the forth metacarpal bone. And like you said it was like a shock and it hurts whenever pressure is applied.

  • Hi, I broke the Middle Distal Third Junction on my Tibia (Comminuted break in 3 like a Y shape) 8 weeks ago, I opted not to have surgery! After 10 days they removed the full leg cast & put me into a PTB (Sarmiento) cast & I’ve had X-Ray check ups every 2nd week up until the 6 week mark where the surgeon said he is happy with how it is looking as it is Aligned & Periosteal has formed on the lateral side & advised he feels comfortable enough now that I can go back in 4 weeks time opposed to the usual fortnightly checkups. He also said if it is still looking good then they will look to remove my cast & put me into a walking boot! How long do you reckon I might be wearing the walking boot? My leg feels totally fine now inside my cast

  • Can stress fracture from overuse repetitive movements over heavy lifting gym weights lead to full displaced humerus fracture its uncommon injury but how did this happen considering being gym consistent with proper diet considering fracture happened from not heavy weights either that time surgeon didn’t offer conservative treatment he offered me only orif for my proximal humerus

  • I did a reconstructive surgery about 3 months ago where they had to implant a rod to help with the union of my bones, a month after the surgery the pains stoped but recently the pains are coming back just Like the once I felt few weeks after the surgery, hope this doesn’t mean the rod shifted or something

  • It’s been 14 weeks since i had the operation for my humurus fracture and it has not been consolidate yet. It’s actually my seconde operation. The first one that i had did not work and my bones did not heal so i had to go through the seconde operation after 4 years of having the fracture. Now that it has not consolidate yet even after 14 weeks of my seconde operation, it worries me sick. Worried that it wouldn’t consolidate. Is it normal for this to happen ? Can it take more then 14 weeks to heal ?

  • Fracture to upper arm, acute pain, not much sleep for first two days.. 10 on pain scale.. Could not sleep on back, painful.. 10 on pain scale.. Slept on sofa, opposite side to fracture, feet on floor, head supported by lots of pillows, lovely to sleep for three hours, painful when moving back to sitting position.. 4 / 5 on pain scale..

  • Hi thanks for the great article! A question about PT, I have a fractured Fibula (Webber b) non displaced from falling down the stairs, my inside ligament is fine! I was casted for 4 days then aircast for approx 6 weeks, weight baring as tolerated, I’m 4 weeks after break but can’t seem to point my foot, I can flex ok though! Doc told me up and down motions only but all articles seem to say I should be doing more exercises. What would your advice be? Thanks all the way from Northern Ireland! ❤️

  • I have a 3rd metacarpal fractured due to a gun shot wound. I can still move all fingers. No pain! It’s been 6 weeks and still in cast. Nerve has been stretched but I still feel everything I touch. I’m hoping I will still be able to work because I use my hands alot. The bullet only had the middle shaft bone.
